CENTER FOR TRANSITION TO ADULT HEALTH CARE 

The Center for Transition to Adult Healthcare is a national health care transition resource center. Their goal is to empower youth and young adults with intellectual and developmental disabilities ages 12-26 to transition from pediatric to adult care.

COMMUNITY OPTIONS, INC. OF NASHVILLE 

Community Options is a national nonprofit that provides community-based options for individuals with disabilities living in Hendersonville and other local areas.  Services include housing, employment, and the School to Employment Program for transitioning youth and young adults

GOT TRANSITION® 

Got Transition helps youth and young adults move from pediatric to adult healthcare, including behavioral healthcare. They have resources for clinicians, youth and young adults, and parents and caregivers.  They offer consultation to clinicians and special education programs.

IDEAL PROGRAM AT LIPSCOMB

The IDEAL Program is a two-year program that offers a college experience for students with intellectual disabilities. IDEAL Summer Academy is a week-long residential experience for students with intellectual disabilities who are rising seniors in high school to age 24.

MILLAR RICH

Millar Rich provides residential, employment, and day services for adults.  Their supported employment program offers youth and adults with Intellectual and Developmental Disabilities pre-employment, job placement, and ongoing support services.

PROJECT SEARCH 

Project SEARCH® is a transition to work program for the purpose of securing employment for people with disabilities.  It is a business-led, one year employment preparation program that takes place entirely at the workplace. Interns participate in an internship program during their last year in high school. Welch College in Gallatin does a school-based model and several sites in Nashville do an adult-based model, including Vanderbilt Children’s Hospital.

TN CENTER FOR DECISION-MAKING SUPPORT

The TN Center for Decision-Making Support provides resources to help people learn how much or what kind of support they might need about choices regarding time, finances, or health.  Supports might include forms, legal documents, or formal agreements.

TRANSITION TN

Transition TN offers free training and resources for educators and Pre-Employment Transition Services (Pre-ETS). They are Tennessee’s online home for training and resources to prepare students with disabilities for life after high school.
